7 ปี หลังจากการตายของ Michael Scofield (นำแสดงโดย Wentworth Miller) ทุกคนต่างก็แยกย้ายกันไปมีชีวิตใหม่ของตัวเอง จนกระทั่งวันหนึ่ง Theodore “T-Bag” Bagwell (นำแสดงโดย Robert Knepper) ได้ถูกปล่อยตัวออกจากเรือนจำ Fox River พร้อมกับได้รับข่าวมาว่า Michael Scofield อาจจะยังมีชีวิตอยู่ในเรือนจำ Ogygia ประเทศเยเมน นั่นจึงทำให้ Lincoln Burrows (นำแสดงโดย Dominic Purcell) และ Benjamin Miles “C-Note” Franklin (นำแสดงโดย Rockmond Dunbar) ตัดสินใจได้ออกเดินทาง เพื่อหาทางแหกคุกช่วยเหลือ ไมเคิล ออกมาจากเรือนจำนั้นให้ได้ ก่อนสงครามกลางเมืองจะถล่มคุกเสียก่อน

ทางด้าน Sara (นำแสดงโดย Sarah Wayne Callies) และ Mike Scorfield (นำแสดงโดย Christian Michael Cooper) ลูกชายของ ไมเคิล ที่อาศัยอยู่กับ Jacob (นำแสดงโดย Mark Feuerstein) ผู้เป็นสามีใหม่ ก็พยายามพิสูจน์ความจริงว่า ไมเคิล อดีตสามีของเธอไม่ใช่ผู้ก่อการร้ายในนาม คาเนล อูทิส แต่ยิ่งถลำลึกลงไปเท่าไหร่ อันตรายก็เริ่มเข้าใกล้เธอและลูกมากขึ้นเท่านั้น
