ในที่สุดสมุนของแอนโทนี่ก็ทำสำเร็จ และเมื่อทอมมี่พยายามหลบหนี เขาก็ถูกตีหัวจนสลบ เมื่อรู้สึกตัวตื่น ทอมมี่ตกอยู่ในกำมือ
ของมาเฟียเยอรมันและทีมแพทย์ผู้เชี่ยวชาญ และแล้วความจริงก็ถูกเปิดเผย ของที่บิ๊กดั๊กสั่งให้ทอมมี่นำมามอบให้มาเฟียเยอรมัน
ก็คือตัวทอมมี่เอง มาเฟียเยอรมันผู้กำลังป่วยหนักต้องการอวัยวะภายในของ ทอมมี่มาต่ออายุให้ตัวเขา เมื่อความตายรออยู่เบื้องหน้า
ทอมมี่ต้องตัดสินใจว่าเขาจะยอมสละชีพต่อเองเพื่อน้อง ชาย หรือต่อสู้เพื่อให้ตัวเองรอดชีวิตดี!!!
